Mentors & Outreach Coordinators

Welcome! This section is for those who are actively involved in the program. We first would like to thank you for taking on this role. As a leader of your school team, we realize that you have many other issues to take on in your community and we know that your time is very valuable. As a mentor of this program, your goal is to keep the students motivated and help to ensure that all activities get done.

This site was created to support your efforts. Here you will find valuable information related to implementing this program in your school and community. We hope this will provide you with many of the resources you need to make your program the best it can be. You will find the following areas of interest:

  • Report forms: these can be downloaded and used. You will also find them in your printed manuals and flash drives. Learn more
  • Additional activities: these will cover Phases I, II and III and will complement the activities already in the manuals. Click here
  • Templates for policies: these include sample policies for schools, restaurants, other businesses and city ordinances. Click here
  • Information on implementing and enforcing polices: these include samples of materials that will help towards implementation and enforcement of policies. Click here
  • Support forms: these will help teams estimate and gather support for local smoke-free policies. These forms are to be used each time you complete an activity. Once completed, send all surveys and completed report forms to your Outreach Coordinators. Click here
